A CAUTI, or a UTI associated with a catheter, is common if you have an indwelling catheter inside your urethra.

Symptoms are similar to a general UTI and include:

bloody or cloudy urine
gritty particles or mucus in your urine
urine with a strong odor
pain in your lower back
chills and fever
There are multiple ways bacteria or fungi can get into your urinary tract and cause a CAUTI:

via your catheter
upon insertion
if your drainage bag isn’t emptied properly
if your catheter isn’t cleaned often or correctly
if bacteria from feces gets on your catheter

HOW TO CARE FOR A CATHETER

To care for an indwelling catheter, clean the area where the catheter exits your body and the catheter itself with soap and water every day. Also clean the area after every bowel movement to prevent infection.

If you have a suprapubic catheter, clean the opening in your belly and the tube with soap and water every day. Then cover it with dry gauze.

Drink plenty of fluids to help prevent infections. Ask your provider how much you should drink.

Wash your hands before and after handling the drainage device. DO NOT allow the outlet valve to touch anything. If the outlet gets dirty, clean it with soap and water.

Sometimes urine can leak around the catheter. This may be caused by:

Catheter that is blocked or that has a kink in it
Catheter that is too small
Bladder spasms
Constipation
The wrong balloon size
Urinary tract infections
POSSIBLE COMPLICATIONS

Complications of catheter use include:

Allergy or sensitivity to latex
Bladder stones
Blood infections (septicemia)
Blood in the urine (hematuria)
Kidney damage (usually only with long-term, indwelling catheter use)
Urethral injury
Urinary tract or kidney infections
Bladder cancer (only after long-term indwelling catheter)
Call your provider if you have:

Bladder spasms that do not go away
Bleeding into or around the catheter
Fever or chills
Large amounts of urine leaking around the catheter
Skin sores around a suprapubic catheter
Stones or sediment in the urinary catheter or drainage bag
Swelling of the urethra around the catheter
Urine with a strong smell, or that is thick or cloudy
Very little or no urine draining from the catheter and you are drinking enough fluids
If the catheter becomes clogged, painful, or infected, it will need to be replaced right away.

says there is a "very effective" supplement called D-Mannose.

"This is the actual active, therapeutic ingredient taken from cranberries. The good news is that it is safe to consume, and can be used by children and pregnant women, and also on an ongoing basis for those suffering from chronic UTI," she says.

Maintaining the
healthy growth of good bacteria in your gut, and your bowel motions are also crucial, Smith says.

"Often, with the chronic use of antibiotics, stress and some vaginal washes and spermicides, the natural growth of good bacteria is compromised, allowing opportunistic bacteria to move into the urethra and cause issues," she says.

"There are specific probiotics available, like L. rhamnosus, L. reuteri, B. bifidus and B. lactis, for example, that are designed to survive the gastrointestinal tract and colonise the colon and vagina.

"They crowd out the other pathological organisms. If the E. coli form of bacterium is not present near the urethra, urinary tract infections are less likely.

"Supporting healthy gut flora might also help in aiding regular, well-formed bowel motions. Many infections are caused by stray bacteria - like E. coli - from the rectum moving to the urethra. This most commonly occurs when one is suffering from constipation or diarrhoea," Smith says.

Good daily habits are also necessary. For example, always wipe from front to back to minimise the transfer of bacteria from the rectum to the urethra.

To keep your immune system strong so that it can fight off the infection, it's important to eat well, exercise regularly and get enough sleep.

What is D-mannose?
D-mannose is a type of sugar that’s related to the better-known glucose. These sugars are both simple sugars —that is, they consist of just one molecule of sugar. As well, both occur naturally in your body and are also found in some plants in the form of starch.

Several fruits and vegetables contain D-mannose, including:

cranberries (and cranberry juice)
apples
oranges
peaches
broccoli
green beans
This sugar is also found in certain nutritional supplements, available as capsules or powders. Some contain D-mannose by itself, while others include additional ingredients, such as:

E. coli bacteria cause 90 percent of UTIs. Once these bacteria enter the urinary tract, they latch on to cells, grow, and cause infection. Researchers think that D-mannose works for a UTI by preventing these bacteria from latching on.

After you consume foods or supplements containing D-mannose, your body eventually eliminates it through the kidneys and into the urinary tract. While in the urinary tract, it can attach to the E. coli bacteria that may be there. As a result, the bacteria can no longer attach to cells and cause infection.

There isn’t much research on the effects of D-mannose when taken by people who have UTIs, but a few early studies show that it might help.

A 2013 study evaluated D-mannose in 308 women who had frequent UTIs. D-mannose worked about as well as the antibiotic nitrofurantoin for preventing UTIs over a 6-month period.

In a 2014 study, D-mannose was compared to the antibiotic trimethoprim/sulfamethoxazole for treatment and prevention of frequent UTIs in 60 women. D-mannose reduced UTI symptoms in women with an active infection. It was also more effective than the antibiotic for preventing additional infections.

A 2016 study tested the effects of D-mannose in 43 women with an active UTI. At the end of the study, most women had improved symptoms.

How to use D-mannose

A lot of different D-mannose products are available. When deciding on which to use, you should consider three things:

whether you’re trying to prevent an infection or treat an active infection
the dose you’ll need to take
the type of product you want to take
D-mannose is typically used for preventing a UTI in people who have frequent UTIs or for treating an active UTI. It’s important to know which of these you are using it for because the dosage will differ.

The best dose to use isn’t entirely clear, however. For now, the doses that have been used in research are recommended:

For preventing frequent UTIs: 2 grams once daily, or 1 gram twice daily.

For treating an active UTI: 1.5 grams twice daily for 3 days, and then once daily for 10 days; or 1 gram three times daily for 14 days.
D-mannose comes in capsules and powders. The form you choose mainly depends on your preference. You might prefer a powder if you don’t like to take bulky capsules or want to avoid the fillers included in some manufacturers’ capsules.

Keep in mind that many products provide 500-milligram capsules. This means that you may need to take two to four capsules to get the desired dose.

To use D-mannose powder, dissolve it in a glass of water, and then drink the mixture. The powder dissolves easily, and the water will have a sweet taste.

Abstract
OBJECTIVE: Urinary tract infections still represent a significant bother for women and result in high costs to the health system. D-mannose is a simple sugar; it seems able to hinder bacteria adhesion to the urothelium. The present study aimed to determine whether D-mannose alone is effective in treating acute urinary tract infections in women and its possible utility in the management of recurrences.

PATIENTS AND METHODS: This is a pilot study, performed between April 2014 and July 2015 at Department of Gynaecological Obstetrics and Urologic Sciences of "Sapienza" University of Rome. A D-mannose compound was administered twice daily for 3 days and then once a day for 10 days. Changes in patients' symptoms, the therapeutic effects and changes in quality of life (QoL) were evaluated clinically and using a specifically validated questionnaire (UTISA). After described treatment, patients were randomized in receiving or not prophylaxis in the next 6 months.

RESULTS: Mean UTISA scores recorded after completing the treatment, compared with baseline scores, showed a significant improvement of the majority of symptoms (p < 0.05). D-mannose seemed to have had a significant positive effect on UTIs' resolution and QoL improvement (p = 0.0001). As prophylactic agent administered for 6 months, it showed promising results (4.5% vs. 33.3% recurrences in treated and untreated patients respectively).

CONCLUSIONS: The results of this study suggest that D-mannose can be an effective aid in acute cystitis management and also a successful prophylactic agent in a selected population; however, more studies will certainly be needed to confirm the results of our pilot study.

Cystitis is an inflammation of the bladder. Inflammation is where part of your body becomes irritated, red, or swollen. In most cases, the cause of cystitis is a urinary tract infection (UTI). A UTI happens when bacteria enter the bladder or urethra and begin to multiply.

The article noted that HAIs increase the cost of healthcare. In the United States, the estimated annual cost of treating just five major HAIs was close to US$10 billion (S$13.7 billion) in 2012. There are no figures for Singapore.

Some of the bugs causing the infections here could have been brought in by the patient but had lain dormant until the person's immune system was breached.

Prof Hsu said a common reason for a breach is the use of catheters, since these allow the bug to bypass the body's usual protections.

The study found that half of urinary tract infections, a quarter of hospital-acquired pneumonia and a fifth of bloodstream infections were linked to the use of devices.

It noted: "This represents a key target intervention area for reducing the HAI (Healthcare  associated infection) prevalence in Singapore, as device-associated HAIs are considered to be largely preventable."

Sarcopenia and treatment

One of the best ways to treat (and indeed prevent) sarcopenia is exercise. Resistance training or strength training – using exercises that increase muscle strength and endurance with weights or resistance bands – has been shown to have a positive effect on our neuromuscular system, hormone concentrations and protein synthesis.

Good nutrition also plays a role. Aim to have a varied diet, rich in fruit and vegetables and good quality protein.

According to the National Dairy Council, not all proteins are the same: “Proteins are made up of building blocks called amino acids, with an amino acid called leucine being one of the most effective at stimulating muscle growth.

Protein sources that are rich in leucine include lean meat, eggs and dairy. Dairy products such as milk, yogurt and cheese are not only good sources of protein but also contain a matrix of other vital nutrients including calcium, riboflavin, vitamin B12, iodine, potassium and phosphorus,” it says.

Finally, although drugs such as testosterone and HRT may have benefits, drug therapy is not recommended for sarcopenia.

以下幾個簡單的飲食核心策略,幫助你在這場人與癌症的戰爭中,提升作戰力。

策略一:攝取優質的蛋白質

蛋白質是構成身體細胞的主要成分,我們的免疫系統當然也需要它才能正常運作。一般人一天需要吃3~5份蛋白質,癌症治療中更需要高到 9-13 份蛋白質,治療中從動物及植物性兩種來源攝取,治療完可以植物性為主。

優質的動物性蛋白質有瘦肉、雞肉、魚肉(特別是含omega-3脂肪酸的深海魚,如鮭魚、鮪魚、鯖魚、秋刀魚等)、海鮮(蚌殼)、低脂奶類、雞蛋等;植物性蛋白質可以吃豆類、豆腐及豆製品、堅果類等,但是治療中的營養蛋白質計算已【豆魚蛋肉】分量計算為主。

豆類及堅果類裡還含有豐富的維生素E。研究發現維生素E也有助於製造抗體,增加T細胞的活性,對於免疫功能低下的老年人特別重要。

海鮮裡(蛤仔、牡蠣)含大量的鋅、鐵,也是維持免疫力的重要營養素。醫學上發現,身體裡缺乏鋅的人很難抵擋細菌、病毒的侵襲。

策略二:每天吃1碗五榖雜糧飯

每天至少吃1碗五榖雜糧飯,或是雜糧麵包、全麥製品。

這些粗糙、未精製的食物裡有豐富的維生素B群和各種礦物質,而精製過的白飯、白麵條、白吐司裡則幾乎沒有這些營養素。

人體需要大量的維生素B群,供應細胞進行增殖、氧化和還原作用,尤其是B2、B5(又稱泛酸)、B6及葉酸和維持細胞黏膜健康及製造抗體等免疫功能有關。

策略三:吃各種顏色的蔬果

一天要吃3種蔬菜、2種水果,種類愈多愈好,簡單的方法是以顏色分辨。

▓蔬菜選一綠、一紅、一黑

每天至少吃1~2碗深綠色葉菜。它們的營養成分多又豐富,有維生素A、B群、C、E及各種礦物質,提供免疫系統 T 細胞正常運作的必要養分。

每天吃1份紅、橘或黃色蔬菜。例如胡蘿蔔、甜椒、南瓜、地瓜、金針、黃秋葵等。

這一類蔬菜裡含有大量的β胡蘿蔔素(水果中的番茄、芒果、紅肉葡萄柚、木瓜、柿子、西瓜、哈密瓜裡也有),進入人體後會轉換成對提升免疫力很重要的維生素A。

另外,適量攝取黑色的香菇及其他菇類。

菇類裡的多醣體(polysaccharide)成分被研究證實具調節、提升免疫功能的作用,因此被視做抗癌的明日之星。它會提高人體巨噬細胞吞噬細菌等入侵者的戰鬥力,而且研究也發現多醣體可以增加自然殺手細胞的數目和活性。

不過香菇的普林含量偏高,患痛風的人應該限量攝取。

▓每天1種高維生素C水果

一天要吃2種水果,其中一種選擇維生素C含量多的。維生素C會刺激身體製造干擾素(一種與免疫功能有關的物質)來提升免疫系統功能。

維生素C含量稱冠的水果是台灣土芭樂,其他高C水果還有泰國芭樂、奇異果、甜柿、木瓜、聖女番茄、草莓、柑橘類水果等,可以每天替換吃。

策略四:喝優酪乳或益生菌

每天喝1~2瓶200~300毫升的優酪乳(或小盒優格)有助維持良好的免疫力。優酪乳主要的功能是調整、改善胃腸道健康。長期、規律地吃優格可以抑制腸道內的有害菌孳生,進一步降低壞菌分泌出致癌物質,減少罹病機會。

近幾年的研究還發現優格中的乳酸菌可以增加由T細胞釋出的γ干擾素(gamma interfer),進一步增加抗體生成;同時,乳酸菌也會加強自然殺手細胞的活動力。

國外研究指出,腸道菌叢的生態,影響免疫治療的成效,佈滿神經淋巴的腸道,健康的菌叢,可以刺激免疫系統,增加PD1/PDL1免疫藥物的反應。

營養專家建議,早上空腹喝1瓶優酪乳,有助刺激腸子蠕動,使排便順暢;另外可以用優格搭配水果,當做下午茶吃。

化療中若怕腹瀉,益生菌(比菲德氏菌-如 洛克飛)是可以減少化療腹瀉的,

策略五:不要吃過量脂肪(含紅肉)

吃太多脂肪會抑制免疫系統功能,身體只需要適量脂肪,就能健康運作。美國麻州大學的一項研究發現,每天的脂肪攝取量從32%降低到23%,可以讓自然殺手細胞的活性增加48%。

一天攝取2000卡的輕度活動量、成年男性,每天可以吃55~66公克脂肪,女性則可以吃45~55公克,不宜超過此量。

同時要注意脂肪的種類,有些脂肪容易抑制淋巴球,減弱免疫系統的作用,例如omega-6脂肪酸含量比例較高的蔬菜油,像玉米油、黃豆油、葵花油等。另外,這一類油脂的性質不穩定,容易在高溫烹調時氧化,產生攻擊免疫細胞的自由基。營養專家建議選擇單元不飽和脂肪酸較多的油比較好,如橄欖油、芥子油等,或者多種油交換使用。

策略六:少吃甜食與糖

單醣類(如葡萄糖、果糖)和甜食會影響人體製造白血球,也影響它的活動力,降低身體抵抗疾病的能力。國外研究報告指出,當人們吃下18~20茶匙(約100公克)的糖,白血球抵抗疾病的能力就會減少50%以上。

建議從少喝含糖飲料著手,改喝不加糖的綠茶、花草茶、水果茶,當然喝白開水也很好,另外,吃甜食也須限量。

策略七:適度補充品、維他命(選擇性)

工作時間長、壓力大又經常外食的人,容易飲食不均衡,補充維他命是必要的。去年美國哈佛大學發表在《美國醫學會期刊》(JAMA)的一篇報告,呼籲每天補充綜合維他命來預防慢性疾病及促進成人健康,營養專家不建議吃單劑的維他命,吃綜合維他命比較好,劑量不需要太高,各種維生素、礦物質不要超過RDA(每日建議攝取量)的100~150%。

均衡飲食下,或有使用腫瘤配方或均衡配方營養品(飲沛、倍速、安素)的人,不需額外再吃綜合維他命,可考慮加強維他命D3,抗發炎薑黃素,菇類多醣體,來增加免疫系統的活性(NK,樹突,T細胞)。

如下圖,薑黃素的抗發炎作用,可幫助增加毒殺型T細胞、增加好的免疫激素路徑【Th1】的表現,減少壞免疫【Treg】細胞的表現,進而幫助活化免疫系統。

策略八:腫瘤配方營養品/麩醯胺酸--免疫營養的角色

均衡飲食外,癌症治療中,我們更需精準的免疫營養配方策略,給身體【魚油、精氨酸、和苷酸、麩醯胺酸】四大免疫細胞所需的營養素,來減少壞發炎,提高免疫細胞的活性,尤其在手術前後、化放療期間或治療完長達6-9個月,會有免疫低下的情況,也是腫瘤細胞有機會逃脫的時間點,加強免疫是非常重要的關鍵。

策略九:中強度的運動

運動,可以減少慢性發炎,增加好免疫(TH1) 自然殺手細胞的活性,是最便宜的自我免疫治療,減少賀爾蒙、壓力激素、發炎激素,與健康飲食搭配,研究中有發揮一加一大於二的功效.

我老了,我感到幸運 。
有的人少年夭折,
有的人壯年早逝,
有的人倒在老年的門檻上,
沒能活到退休。
能夠進入老年,
走過完整的人生路,
享受上蒼賜予的天壽,
經寒暑曆春秋,
這是幸運眷顧了我。
我感恩,我知足。

我老了,我感到自由。
不再求升遷,
不再需職稱。
拋棄了奢望,
減少了欲求。
告別職場,
卸下了工作。
一切行動自作主,
擁有了支配自己時間的自由。
來來去去無約束,
灑灑脫脫為自己活。

My experiences with health supplements

*I don't have teeth and gum problems and mouth ulcers, hand eczema anymore* after I re -started to take the following regularly every day before 2359 hours 2 years ago:

1. Multivitamins,
2. Vitamin C 1000mg
3. B Complex 100,

Now I also take
4. OMEGA 3 fish oil
5. Biotin 10000mcg (for hair and skin, finger nails. Very good)
6. zinc 15mg
7. Other less important health supplements.

*My overall immune system is much better too. All thanks to the health supplements I take.*

Sometimes I feel getting sick,  just take Panadol, take a good sleep and get well again.

CONCLUSIONS:


Higher circulating 25(OH)D was related to a statistically significant, substantially lower colorectal cancer risk in women and non-statistically significant lower risk in men. Optimal 25(OH)D concentrations for colorectal cancer risk reduction, 75-100 nmol/L, appear higher than current IOM recommendations.

The role of calcium, vitamin D and phosphorus in maintaining healthy teeth

Calcium, vitamin D and phosphorus all play a vital role in the formation and maintenance of healthy teeth and gums in both children and adults.

Calcium plays a role in making the jaw bones healthy and strong to hold the teeth in place. However, calcium needs phosphorus to maximise its bone strengthening benefits. Children’s teeth need adequate calcium and phosphorus to form a hard structure during growth.

Vitamin D regulates the body’s balance of calcium and phosphorus and can promote absorption. Vitamin D can also help to decrease inflammation of gums which is associated with periodontal (gum) disease*.

A healthy diet is essential for healthy teeth. The table below shows just some of the foods that contain calcium, vitamin D and phosphorus.

抗癌不能瘦,補充營養品增肌肉

抗癌長路漫漫,「體重」與「營養」是關鍵。安南醫院放射腫瘤科主治醫師詹凱翔提出三項重要方針:抗癌要贏、體重要行、肌肉要增!病友千萬別想餓死癌細胞,因為正常細胞搶不贏癌細胞,有可能傷敵一百、損己一千。

詹凱翔表示,12年前岳父罹患食道癌第三期,至今控制良好,關鍵在於體重與營養。癌症患者整體營養不良率高達四成,尤其是頭頸及腸胃道癌患。如何判斷患者營養是否足夠?可以從明顯體重減輕及早發現。

「癌友補充營養就像儲蓄,在進入療程前,應大量存本,努力增重(肌肉),治療後才有本錢恢復至原本體重。」詹凱翔說,均衡攝取碳水化合物、蛋白質、脂質、維生素、礦物質、水,其中以蛋白質最重要,是增加肌肉重要來源。

詹凱翔提醒,肌肉是對抗發炎的利器,治療期間患者應攝取足夠熱量,增加肌肉維持體重,累積接受治療的本錢。如果患者體重半年下降10%、一個月下降5%,可能是惡病質或惡病質前期,要特別注意。

臨床研究建議魚油攝取量為每天2克EPA。在新鮮食物中,2克EPA大概等於1.4公斤鮭魚,或96尾蝦,不容易從飲食達到足夠量,可補充癌症專用營養品,並搭配阻力有氧運動,提升患者生活品質。

高雄榮總營養師施水鳳表示,營養不良患者使用口服營養品,可提升生活品質,幫助體重維持。癌症營養攝取量可根據歐洲營養醫學會建議,患者每天應攝取25-30大卡/公斤熱量及1.2-1.5克/公斤蛋白質,例如:體重60公斤患者,每天應攝取1800大卡熱量與90克蛋白質。

施水鳳說,癌患食欲普遍不佳,特別是治療中引起的口腔潰瘍與噁心,讓癌友愈來愈不想進食,此時建議口服營養品介入,但要注意的是,過高的脂肪攝取容易產生飽足感,影響食欲,患者可能還沒補充足夠的營養素,就因飽足而無法繼續進食。

若患者能正常飲食最好,吃適量食物刺激胃口更好。建議癌友少量多餐,選擇開胃食物如山楂、陳皮、洛神茶、四神湯等,並同步補充口服營養品,爭取更好的治療品質。

世界卫生组织称, 个人的健康和寿命15%决定于遗传,10%决定于社会因素,8%决定于医疗条件,7%决定于气候影响,60%决定于自己。
