There are three different types of ACP forms:
1. The General ACP form, which is used when a person is generally healthier or has early stage chronic diseases;
2. Disease-specific ACP, which is used for persons with progressive, life-limiting illnesses, suffering frequent complications;
and
3. Preferred Plan of Care, which is used for persons who are at the end-of-life stage.
Depending on your health status, the ACP facilitator would advise on which is a suitable form to use.
