Action plan to help older Singaporeans live well as they age and work longer

SINGAPORE – Singapore will reach out to more than half a million of its older adults over the next five years through a programme that empowers them to improve and maintain their health as they age.

The Live Well, Age Well programme, run by the Health Promotion Board and People’s Association, is part of the Republic’s 2023 action plan for successful ageing, which also includes efforts to keep older people employed for longer and stay connected with their loved ones.

The revised plan, an update of the 2015 Action Plan for Successful Ageing blueprint, was launched on Monday by the Ministerial Committee on Ageing – helmed by Health Minister Ong Ye Kung – at the Ci Yuan Community Centre in Hougang.

Mr Ong called ageing “the most significant social development in Singapore” as the nation redoubles efforts to enable its seniors to lead healthy, secure and more fulfilling lives.

It is estimated that by 2030, one in four people here will be 65 and above, as Singaporeans are living longer and having fewer babies. More seniors will also be living alone.

The Live Well, Age Well programme, which targets people aged 50 and above, offers activities such as group exercise sessions and educational sessions on mental well-being. It is being gradually introduced in community clubs and active ageing centres.

Mr Ong said he hopes older adults will also enrol in the Healthier SG initiative, which starts this year and emphasises preventive care such as linking seniors with a family doctor who will help develop a personalised care plan for them.

Under the action plan, a credit scheme to support the hiring of older workers will be extended to let seniors work longer by providing wage offsets for companies that employ workers aged 60 and up who earn up to $4,000 a month.

The Government will also extend the part-time re-employment grant, with revised criteria, from 2023 to 2025.

Singapore raised the retirement age of workers here from 62 to 63 and the re-employment age from 67 to 68 in 2022. The target is to increase the retirement age to 65 and the re-employment age to 70 by 2030.

The planning for an ageing population started years back.

In 2015, when one in eight people was aged 65 and older, the Government unveiled the $3 billion Action Plan for Successful Ageing after it sought the views and aspirations of Singaporeans on the issue.

That plan focused on creating opportunities for seniors to learn, volunteer and live independently.

The 2023 revised action plan focuses on community initiatives.

It aims to keep abreast of the evolving needs of a growing pool of older adults and comes after 40 engagement sessions involving more than 5,000 Singaporeans.

It has three thrusts – care, contribution and connectedness – to help Singaporeans live life to the fullest.

Care is about empowering seniors to take charge of their health through preventive measures, active ageing programmes and care services, with efforts on managing dementia, and helping more Singaporeans live out their lives at home rather than in hospitals.

Seniors will also get support to contribute their knowledge and expertise, learn continuously and engage in volunteerism.

They will be able to stay connected with society and their loved ones as more therapeutic gardens are set up, for instance.

Speaking at the launch of the action plan on Monday, Mr Ong said: “While ageing is a demographic fact, rapid deterioration of health need not be a given… Covid-19 has taught us that with the right preventive steps – through diet, exercise, wearing a mask when you have to, regular health screening, staying mentally healthy and happy – we can stave off frailty and severe diseases for as long as possible.”

At the same time, there is also a need to strengthen support systems to enable ageing in communities, rather than in institutions.

He said: “In communities, our seniors are more likely to have friends, loved ones and activities to engage in. These are the best therapies to help them stay healthy and avoid frailty.”

Mr Ong also held a panel discussion with Minister for National Development Desmond Lee, Minister for Manpower Tan See Leng and National Trades Union Congress secretary-general Ng Chee Meng, who are all members of the Ministerial Committee on Ageing.

“The most encouraging aspect of the successive action plans for successful ageing is the fact that they emphasise ground-up initiatives and collaborations, and active participation by all,” said Mr Ong.

He said the report does not contain a typical narrative of only what the Government is doing to promote successful ageing, but also includes what society is doing.

“You will find examples of programmes implemented not just by ministries and government agencies, but also by corporations, charity organisations and communities. Ageing is, after all, a societal phenomenon requiring a societal response,” he said.

本地聘请住家陪月嫂的费用一般介于2800元至3800元,豪华月子中心入住28天要2万多元,一般有客房、育婴室、瑜伽室等设施和按摩护理和医美疗程等服务。

有越来越多本地和外地人愿意花费两万多元,大手笔体验豪华坐月子服务。月子中心越开越多,有的甚至设在酒店内,或者走精品酒店风格,让客户享受酒店级设施与照料。

在本地聘请住家陪月嫂的费用一般介于2800元至3800元,相比之下,豪华月子中心如Kai Suites入住28天要2万多元。Kai Suites位于杜尼安路,这里有18间客房、育婴室、瑜伽室,并提供按摩护理和医美疗程等服务。

另一家月子中心Confinement Angels让妈妈入住香格里拉服务公寓,收费1万8888元起。

去年9月投入运作的My Queen是本地首个与酒店合作设立的产前产后护理中心,位于圣淘沙豪亚度假酒店(Oasia Resort Sentosa)。

中心是酒店的承租人,目前试行阶段向酒店预留四间可扩大的高级客房和一间大套房,价格从1万7000元起跳。中心开幕至今,有三对产后夫妇入住,都是本地人。目前共八人的护理团队包括资深哺乳顾问,全天候照看宝宝。

爸妈们能使用健身房和游泳池,妈妈可以参加产后瑜伽、水疗和手工艺等,还请来专业摄影师拍摄家庭照。除了酒店早餐,中心还提供月子午餐和晚餐。

My Queen创办人张美玲接受《联合早报》访问时说,中心主要采用有科学证明的方式,协助产妇康复,但也尊重传统坐月子的习俗,因此会准备中药泡澡水等护理。

“传统坐月子一般是请陪月嫂,但很多新手爸妈不一定赞同陪月嫂的所有做法,却又没有其他可靠咨询管道。但是,中心的专业团队有医疗知识。”

中心自去年首季开始与南洋理工学院合作,由学院给中心的护理人员提供基本婴儿护理等培训,并协助分析坐月餐的营养。

李雪华(31岁,家庭主妇)在去年10月生产后,决定预定My Queen的28天套房入住配套,主要因为中心远离都市喧嚣,仿佛在国外度假,她花了近2万6000元。

她说:“请陪月嫂全天候陪伴,我觉得没有私人空间;有些坐月中心租一间房间给我们,但活动空间比较小,像在自己家里,感觉很有压迫感。考虑到下来得自己照顾孩子,我们希望在坐月子的时候先好好放松,就选择入住以酒店度假为概念的月子中心。”

2021年3月在实乞纳一带设立的月子中心幸孕阁(The Clover Suites)把旧店屋改造成类似精品酒店的布局,结合中西方健康模式,专业团队包括哺乳顾问、产后和哺乳按摩师、新生儿保姆和营养师,提供全天候照顾。

幸孕阁有九间豪华客房,也提供产前产后按摩、美容水疗和美发沙龙等服务,以及高级咖啡厅。14天配套从7888元起跳;28天则从1万3888元起跳。

中心发言人受访时透露,近期有越来越多本地父母入住,2021年的本地客户占两成,外国客户则占八成,包括中国大陆、台湾、马来西亚和韩国人。

“如今本地客户已增加至约占一半的总入住率。中心至今年6月的预定已满。”

考虑到坐月服务在多个亚洲国家越来越受欢迎,中心认为,这类酒店式的坐月服务需求将继续增加,因此正寻找机会开设更多分行,同时探讨开设学院,培训新手父母和有兴趣成为专业保姆的人。

SINGAPORE – As dementia cases rise with a rapidly ageing population, memory clinics that can confirm the diagnosis and cause of the ailment have been set up in 16 of the 23 polyclinics here to improve access in the community.

The Ministry of Health (MOH) is bumping up the number of community outreach teams known as Crest – which identify seniors at risk and refer them for assessment – from 61 to 73 by March 2025.

At the same time, the number of Crest post-diagnostic support teams that help newly diagnosed dementia patients and their families cope will be tripled to six, under a pilot. These teams link the families to resources and equip caregivers with coping strategies.

These initiatives are in a 2023 action plan for successful ageing, launched on Monday by the Ministerial Committee on Ageing, of which Health Minister Ong Ye Kung is chairman.

They add to a host of existing ones that the Government has put in place over the years, as it aims to build a dementia-inclusive Singapore. As part of the 2015 action plan for successful ageing, the Government has already established 15 dementia-friendly communities in neighbourhoods such as Yishun, Bedok and Toa Payoh East.

An estimated one in 10 people aged 60 and above here has some form of dementia, a syndrome in which there is deterioration in brain function. It is not a normal part of ageing.

The action plan also states that funding will be provided to enhance research on dementia, catalyse new innovations and scale up solutions shown to have good outcomes in delaying or managing the condition.

“Prevention remains the key pillar of our dementia strategy,” Mr Ong said on Monday at the launch event held at Ci Yuan Community Centre in Hougang.

Many developed countries, like Britain and France, have been able to lower their age-specific incidence of dementia, so the trend is not a given and can be reversed because of preventive health, he said.

This is why Singapore’s major preventive health strategy Healthier SG is a critical and necessary imperative, even for the prevention of dementia, he said.

  • 每天接纳着新加坡垃圾灰烬和非焚化废料的实马高岛,预计将在2035年完成我国首个岸外垃圾埋置场的使命。

然而,这个距本岛南部8公里的垃圾岛,早在1844年就有人居住,1970年前后岛民一度多达近千人,是俗称“海人”(Orang Laut,马来文,意指海人)的岛民的天堂。这些岛民在1977年因国家发展需要,被迫迁移到本岛,分散在直落布兰雅、西海岸、裕廊和亨德申的组屋区。随着他们渐渐年长老去,岛上生活记忆也随之消失。

本报记者走访一些前岛民,以马来语、福建话、英语和华语,聆听岛民生活点滴,让更多人认识这特有的新加坡文化。

“那里是新加坡再也找不到的人间天堂。”

这是个没有车辆的小岛,从码头到岛中央得步行至少30分钟。这里没有一开即流的自来水,也没有电流、电灯和风扇,更没有全天候的诊所和餐厅。但是,屋子后院是天然大浴场,随时可以钓鱼或捉螃蟹;岛上所有小孩不用上游泳课也会游泳;白天在简陋的屋外捡果子、爬树或捉蚯蚓;晚上点起蜡烛,提着一壶甜甜的茶,与家人躺在码头甲板上边聊天边细数天上繁星;每天被蚊子叮、与苍蝇分享盘中餐、饭前不洗手或是赤脚满村跑,也鲜少生病。

每个前岛民受访时回忆起当年的实马高岛(Pulau Semakau)日子,语气格外激昂兴奋,即便已离开近45年,各种生活便利和现代化始终无法取代岛上生活的无忧与快乐。

谢家九兄弟姐妹是岛上少数华人家庭

68岁的谢明锦和其他八个兄弟姐妹出生于实马高岛,是岛上少数的华人家庭。

19世纪初,曾祖父母从中国潮汕南来到实马高岛,经营白石灰生意,是岛上少数聘请工人、拥有土地的富裕家庭。出生于新加坡本岛的祖母在年轻的时候,还有佣人服侍。

由于海水侵蚀,岛的面积越变越小,打击白石灰产量,也淹没了当时的墓地。当谢明锦的祖父接手白石灰生意时,好景已不再。等到祖父在1943年去世时,全家只靠父亲捕鱼勉强维持生计,不时得要已嫁到本岛的姑妈救济。

实马高岛(Pulau Semakau)还没开发前,是一个状如火焰的红树林小岛,横宽约1公里,南到北端约2.1公里,实马高则源自马来文“ bakau”(意指红树林)。据国家图书馆的一份资料,最早记录实马高岛人烟活动是1844年,当时一个村民遭海盗杀死。

岛的西部是马来渔村,西南部是小小的华人村,岛民多从事捕鱼或务农。1977年所有岛民迁移前,估计岛上约有六七百人,华人有10户,多半是姓谢的潮州人,不少与谢明锦有亲戚关系。

岛上物资匮乏,谢明锦和八个兄弟姐妹自小节俭,连买颗糖果的钱也没有。但他们懂得如何从大自然“找吃”——晚上提灯到海边捉螃蟹;白天在浅滩用短短的镖来捉虾;用镖瞄准退潮后浅滩的乌贼,失准时还被乌贼的墨溅得全身黑。

她们也种木薯、空心菜等卖给岛上马来居民;或在屋旁空地养鸡鸭;或是在上课前趁老师未进课室,悄悄把自己种的水果卖给同学赚点小钱。她们每天与海为伴,熟谙潮汐起落和各种最原始的捕鱼窍门。“我们每个都被晒得黑黑的,搬来本岛时,人家还以为我是印度同胞。”

本岛护士每周到岛上看诊一两次

七岁那年,谢明锦和兄长一样,到岛上唯一的学校——实马高岛马来学校上课。小学以马来语授课,一个年级一班,每班人数不足15人。学校教师和校长每天往返本岛和实马高岛,遇上大风浪无法开船,就在岛上简陋的公务员宿舍留宿。

岛上小孩几乎都不穿鞋,屋子大门鲜少关上。为让岛民与现代化挂钩,岛上建有联络所、诊所和回教堂。本岛护士每周来实马高岛看诊一两次,但在谢明锦的印象里,只记得到诊所帮父亲讨头痛药,自己和周围伙伴什么病痛也没有。偶尔病了,就服用祖母的唐山药箱内不同苦味的药,还相当见效。再不行,就找岛上的马来巫师治病。不少马来岛民病了,也会向谢明锦的祖母讨些中国寄来的药吃。

“我的祖母很受马来岛民爱戴,他们都尊敬她,赞她人好。”

1977年搬到本岛时,她父亲当时已在毛广岛工作数年,子女陆续外出工作或结婚,生计暂时还能应付。但没有工作的谢明锦姐妹却适应不来。“每天看着墙壁,没有工作,没钱出门,很不习惯。”

即使是向来待在家里的母亲也调适不过来,本来天天吃鱼,却突然吃不下任何海鲜。

“在岛上,所有鱼虾都是鲜活的;但本岛的巴刹卖的全是死的,她看了实在吃不下。除非买到非常新鲜的鱼,否则她完全不吃。”父母在2007年和2008年相继过世。

搬到本岛一年多后,谢明锦结婚生子,在家照顾身体不好的家婆。1985年她在惹兰红山租了摊位卖鱼,之后又辗转当搬运工人、在家缝被子、袋子、布花等补贴家用。现在子女各自成家立业,也算苦尽甘来。

她感叹说,本岛虽然生活便利,但样样都需要钱,生活担子沉重多了。

离开多年后,谢明锦曾在实马高岛未改建成垃圾岛前坐船绕岛一两次,可是童年生活的乐园已面目全非、杂草丛生。

“码头没了,以前海边的浮脚屋也拆掉,很多树,一切变了。”

就算是从中学起每天往返本岛的哥哥谢明强(69岁),也不能马上调适过来。“开始时很想念岛上的生活,尤其是大海和钓鱼。而且最大的问题是不懂得买海鲜,因为卖的都是死鱼。”

岛民的求学之路是耗时费力的。像谢明强在小六会考期间,必须到本岛的学校过夜几天应考。中学时,他每天凌晨4点起床,先坐半小时的舢板到毛广岛,再转搭半小时的渡轮到本岛。上岸后,乘坐巴士到位于金文泰的中学。回程也是如此,1时半放学,最快下午4时才能回到实马高岛。如果不是寄住在码头附近的亲戚家,他还得步行30分钟才到自己的家。

这份坚持和毅力,让谢明强顺利加入武装部队,退休后在直落布兰雅开小杂货铺子。

个案② 捕鱼高手重返岛上过淳朴生活

罗哈尼的父母拉尼欧马(已故)和尼娜(88岁)可说是岛上的最后一户岛民。她的外甥菲道尔是倡议保留海人文化和饮食特色网站oranglaut.sg的发起人。

话说1977年,拉尼欧马一家12口搬到直落布兰雅一房式租赁组屋后,不到两个星期,拉尼欧马就决定重返海上生活。

当时实马高岛尚未展开工程,岛上空无人烟,只剩下空置的木制建筑物。拉尼欧马捕完鱼后,就到岛上诊所或码头栖身,几个星期才回本岛探望家人。12年后,当子女都长大了,尼娜也跟随丈夫回到实马高岛生活,成了岛上唯一的住户。

69岁的罗哈尼以马来语说:“除了捕鱼,我父亲没有其他一技之长来养活10个子女。而且这么多人挤在一起,和岛上辽阔的空间差别实在太大。”岛上生活淳朴简单,只要不奢求,捕什么吃什么,就能养活全家。

拉尼欧马是捕鱼高手,知道茫茫大海的哪一处能捕获哪类鱼,捕获后就卖给华人鱼贩。卖剩下的或是孩子钓到的,尼娜用辣椒来煮或用盐巴来烤,当成一顿配饭菜。

想吃菜时,赤脚步行半小时到岛中部,向种菜和水果的华人岛民,以捕获来物物交换。遇到雨季没捕获,菜农也不让罗哈尼空手回家,并且还担心罗哈尼一家不够吃而多给。

1963年至1966年马印对抗(Konfrontasi)期间,岛上的马来人和华人没有发生过任何互相攻击事件。当时村长严厉警告所有村民,谁先动手,不论肤色一概严惩。罗哈尼至今还和岛上华人互访联系,包括岛上杂货店店主林成发(即个案三提到的林珍如的父亲)。

大家不分肤色 亲如兄弟姐妹

罗哈尼说:“岛上没有肤色之分,大家就像兄弟姐妹般帮来帮去,我有许多华族好友。有时好多天无法出海,华人就会和我们免费分享菜啊、水果啊。过年过节,大家交换食物,我们也不会介意吃华人祭祖的祭品。”

岛民是环保实践者。罗哈尼住的浮脚屋,屋顶由椰叶盖成,窗户是上下两条木条串着麻布袋制成的。当拉尼欧马得知毛广岛的朋友搬家,就把朋友家的锌片和木板拆下,用船载回实马高,为自己的家添上屋顶和木制窗。后来,罗哈尼一家迁移到本岛时,这些盖房材料被印尼廖内的亲戚拆下,搬到廖内再使用。

出海以外,岛民没闲着。修网、采集烹煮用的木材、把椰叶梗制成扫把、修屋、种菜、盛雨水等,双手歇不下来。

除了生活习惯不同,岛民使用的一些马来词汇也与本岛不同,导致他们产生差人一等、觉得自己文化修养和知识水平不如本岛人的自卑心态。

然而,正如罗哈尼和妹妹诺爱尼(63岁)所说,本岛生活虽便利,但没钱举步皆辛;在实马高岛,简单过活,心情舒畅,贫乏的生活条件是珍稀的专属经历。太想念岛上生活,就去峇淡岛的亲戚家小住,回味当年岛上感觉。

“那是人间天堂。”

个案③ 杂货店予人方便 有岛民赊账30年归来还钱

83岁的许美玉出生在新加坡本岛,18岁经媒妁之言嫁到巴歪岛,住不到四年,政府收回巴歪岛作为军事区,碰巧丈夫得知朋友要转让实马高岛的杂货店生意,就接手经营,是岛上当时仅有三家杂货店之一。

他们一住就15年,直到1977年被迫搬迁到直落布兰雅组屋楼下商店,现由小女儿林珍如继续经营美宝商行。

林珍如因为念书的关系,平常住在本岛,只在周末和学校假期回岛上生活。即便如此,她对岛上生活却缅怀无限。

“那是新加坡体验不到的生活方式,那里苍蝇和壁虎特别多,退潮时可以看到海滩的海星,晚上躺在码头喂蚊子看星星。我还跟着哥哥姐姐用薯粉泡水倒到海里捉鱼。”

杂货店开业多年 未有过偷窃事件

岛民民风淳朴,许美玉以福建话细数点滴时说,开业多年,杂货店不曾发生过偷窃事件。

不仅如此,林珍如记得一名前马来岛民,一天忽然出现在店里,原来是把30多年前在岛上赊账的20多元欠款,亲自还给父亲。

“这些渔民并不富有,常是以物物交换或赊账的方式到店里换些日常用品,但他们的人情味很浓,也很肯互相帮忙。”

许美玉说,除了开店和家务活儿,她也养鸡卖蛋、摘些水果卖。岛上没水没电,就物资而言,毛广岛就先进多了,因此岛民得到毛广岛码头,装满一桶桶的饮用水用船载回来。

许美玉说:“记得有一回遇到大风浪,浪头比我们的船高很多,天色很暗,看不清方向,船里都是水,我拼命地舀起来,倒到海里。我老公把装满水的桶都丢进海,减轻船的重量。还好遇到海警为我们照明开路。之后,我能不坐船就尽量不坐。”

搬到本岛,许美玉适应得还不错。“有水有电当然方便。只是这里不可能像我以前在巴歪岛般无忧无虑,能用镖捉鱼,现烤现吃。”

依海而生的海人经历,就快随着这代人的老去,淹埋在层层垃圾底下。
